[Forminator Pro] I can't get a Form from Forminator Pro to show in Avada Theme and Buider

I'm trying to use the shortcode to display my form from Forminator Pro in my Avada Theme but it won't display. I can preview in Forminator but the page won't display it.

I'm using Avada Version: 3.7.4 by ThemeFusion
